2020-2021 MPSD Online Registration
MPSD is implementing an online registration process for the 2020-21 school year. This process will allow us to complete and submit the required paperwork and documents through a Google form. This process will take about 30 minutes to complete. This registration form may be completed on your mobile device or computer.
Online Learning Information
All students who are enrolled in MPSD are eligible for blended or online learning. A blended (hybrid) learning model combines face-to-face and online (virtual) instruction into one learning experience for students.

STEP 1: GATHER DOCUMENTATION
You will be asked to upload documentation that your child's school must have prior to official enrollment. If you would like to submit your child's documentation as you complete this form, it may be easier to gather the information listed below prior to beginning the process. You are not required to upload these files in order to complete this pre-registration form, but the documentation must be received by the school prior to official enrollment.
Items required for enrollment and registration in the Meridian Public School District:
- State Certified Birth Certificate - new students only
- All new students (K-12) and incoming 7th graders must provide a valid immunization certificate provided by the MSHD.
2- Proofs of residency
The parent(s) or legal guardian(s) of a student seeking to enroll must provide the school district with a minimum of 1 document from each group below (Group I and Group II) as verification of their address. All documents must be current.
Group I (all students)
- Mortgage Statement
- Property Tax Bill
- Filed Homestead Exemption Application Apartment or Home Lease
- Filed Property or Warranty Deed
Group II (new students and returning students who have moved)
- Driver's License/State Issued ID
- Utility Bill
- Gas Bill
- Garbage Bill
- Electric Bill
- Water Bill
- Cable Bill
- Certified copy of the filed petition for guardianship/final guardianship decree
*ALL BILLS IN GROUP II MUST BE DATED WITHIN 30 DAYS OF STUDENT'S REGISTRATION.
If utilities are included in the Apartment or Home Lease, please use one of the following documents for the second proof of residency (dated within the last 30 days):
- Doctor, dentist or hospital bill (mailed)
- Credit card statement
- Insurance policy (Health)
- State or Federal benefit check
- Salary check stub
- Government mailing (Social Security or Medicaid Determination letter, IRS, etc.)
- Bank Statement
- Driver's License/State Issued ID
MPSD District Administration may approve other documentation that will objectively verify residency, ONLY in circumstances that building level administrators deem extenuating circumstances.
*AUTOMOBILE REGISTRATION AND VOTER PRECINCT IDENTIFICATION ARE NOT ACCEPTABLE PROOFS OF RESIDENCY.

STEP 3: REVIEW INSTRUCTIONS FOR UPLOADING FILES
You will have the option to upload registration documents in certain questions. When you select "Add a File," you will see the option to select files from your mobile device or computer. At this point, you can either take a photo, choose a photo you've already taken, or browse your device for what you want to upload. We recommend completing the form on a cell phone or tablet due to the ease of taking/adding photos of documents.
